2023 Club Reporting Information

Complete one per club of the Club Statistical Report Form (Word file),

Complete one per club of the Club Information Form (Word file), and complete as many ★★ individual reporting forms ★★(each form is a Word file) as needed.

★★ As of November 2023, the reporting zip file has one updated file for Leadership reporting.  If the version of the zip file on your computer is before November 2023, please download this new version.★★

The individual reporting forms will be found in the “downloads” folder on your PC in a “compressed zipped” folder.

Emailed documents are preferred. See the Club Information Form for submission instructions.

Reports need to be emailed or postmarked on or before January 31, 2024.
